I hereby consent to and authorize the use and reproduction of, in print, video, or electronic format by Commonwealth Catholic Charities (CCC), all and/or any part of my story and photographs/video of me, for promotional and educational uses, without compensation.

I agree that CCC may use such information about me or photographs/video of me with or without my name for any lawful purpose, including, but not limited to, printed or electronic publications, web sites, or other electronic communications.

I hereby waive any right to inspect or approve the finished photographs or electronic matter that may be used in conjunction with them now or in the future, whether that use is known to me or unknown, and I waive any right to royalties or other compensation arising from or related to the use of the image.

I further hereby release Commonwealth Catholic Charities from any and all legal liability that may arise from CCC’s use and reproduction of the information, photographs, and video for the purposes outlined herein.
