Since
February 1992, the Home Care Program
of Commonwealth Catholic Charities
has provided case management services
to those living with HIV/AIDS in the Richmond area.
A Home Care case manager monitors
the functioning level of the individual,
assesses their needs and assists
them with obtaining appropriate
services. These include nutritional
supports, financial assistance,
housing, transportation, nursing
care, and counseling.
Individuals and families seeking
assistance can be helped through
funding by Medicaid.
